Six easy steps to nominate
- Download & Print the Proxy Form - sadly there's no electronic submission of OC proxy
- Enter your
- Name(s) of lot owners
- Address
- Lot Number - you'll find your Lot number on the meeting invite from Whittles (email is usually from info.melbourne@whittles.com.au or from an email ending in whittles.com.au) and by clicking the click here to vote link in the body of the email. When the vote page is displayed, your LOT number will be shown in the top right hand corner.

- In the Authorise section, enter the name and address of the person you authorise. You can nominate someone you know who will attend the OC meeting or one of the following:

- Mark Hooper 2801/80 Lorimer St Docklands 3008
- Ken Harvey 2801/80 Lorimer St Docklands 3008
- In the As my/our proxy: section we suggest you tick the first box which says 'to attend, speak and vote in person on my/our behalf until (insert date until which proxy authorisation will be valid, up to a maximum period of 12 months)' and enter the date one year from today.
We suggest this one year nomination as you don't want to have to fill in this form every 3 months when a meeting is held!! - Sign, Print your Name and Date the form
